Geisinger is the latest major health system to partner with Noteworth to connect its clinicians with patients and their health data.

On 25 May, Geisinger announced the launch of ConnectedCare365, a new “care-delivery model” for patients with chronic diseases like diabetes, heart failure and hypertension. ConnectedCare365 will improve health outcomes, increase patient engagement, improve access to care, and lower costs, according to Geisinger.

The platform uses Noteworth’s artificial intelligence and predictive analytics tools to analyze clinical data in electronic health records as well as data provided by the patient through a mobile app, such as weight, blood pressure and glucose levels.

The platform communicates directly with each patient’s care team to help clinicians triage and prioritize care for high-risk patients to prevent unnecessary emergency room visits or hospitalizations.
